The SIBO Preparation Protocol: 5 Essentials to Support Your Gut and Liver

One of the most common mistakes people make when tackling SIBO (Small Intestinal Bacterial Overgrowth) is approaching it like a ‘seek and destroy’ mission. We often feel so desperate to get rid of the bloating, brain fog, and irregular bowel movements that we want to jump straight into the "kill phase."


However, jumping into antimicrobials—whether they are natural herbs or antibiotics—too soon can actually make you feel worse. Without the right foundation, you run the risk of intense die-off reactions, toxic overload, and increased inflammation.


The SIBO 5-pillar preparation protocol


In this guide, I’m walking you through my 5-pillar preparation protocol. This is the exact systematic approach I use with my clients to ensure a smoother recovery and, most importantly, to reduce the chances of SIBO coming back.

Why prep before you 'kill'

When SIBO bacteria die, they release Lipopolysaccharides (LPS)—endotoxins stored in their cell walls. If your gut lining is "leaky," these toxins slip into your bloodstream and overload your liver.


This is known as a Herxheimer (die-off) reaction. It feels like a "SIBO hangover"—intense brain fog, flu-like fatigue, and a worsening of your digestive symptoms. By strengthening your body’s ability to detox and eliminate before we start treatment, we can minimise these reactions.

The 5 Pillars


I recommend following this protocol for 2–3 weeks before starting antimicrobials. If you are highly sensitive, focus on this phase until your body feels stronger.

Pillar 1: Gut Integrity (The Barrier)


We need to ensure your gut wall is strong enough to keep inflammatory LPS toxins inside the digestive tract so they can be flushed out properly.


The Essential: L-Glutamine


  • Why: This amino acid helps "seal" the tight junctions of the gut wall, acting as a protective barrier.


  • My Recommendation: Wonderfoods or My Way Up Vegan L-Glutamine.


Pillar 2: Liver Support


Your liver is the primary detoxifier. If your bile flow is sluggish, toxins just recirculate in your system.


The Essential: Glutathione, Milk Thistle, & Artichoke


  • Why: Glutathione neutralises toxins, milk thistle protects liver cells, and artichoke leaf improves bile flow (which is also vital for managing constipation).


  • My Recommendation: Ultra Nature Liver Detox & Digestive Support. 


    *Note: If you are highly sensitive, stick to a single ingredient like Nature's Sunshine St Mary's Thistle.


Pillar 3: Nervous System/Detox Support

Healing happens in the "Rest and Digest" (parasympathetic) state. Additionally, SIBO often overwhelms the body’s sulfation pathways, which are key to healthy detoxification.


The Essential: Epsom Detox Bath Salts


  • Why: Epsom bath salts consist of magnesium sulfate making it an excellent choice for supporting liver pathways while calming your nervous system. This compound is absorbed efficiently through the skin bypassing any need to be able to absorb through the gut which is important when digestive processes are compromised by SIBO.


This is also the perfect option when you suspect you have sulfur intolerance and in fact, helps to ameliorate issues with sulfur.


  • The Routine: Soak 3 times a week (2 cups in a bath or foot bath).


  • My Recommendation: Amazing You Epsom Detox Bath Salts.


Pillar 4: Cellular Energy (The Fuel)


The detoxification process is "expensive" for your body—it requires a lot of energy to function optimally.


The Essential: Activated B-Complex


  • Why: "Activated" means the vitamins are in their most bioavailable, usable form. B vitamins are essential for liver function and are very often depleted in those with SIBO.


  • My Recommendation: MicroNutrition Advanced B Complex.


Pillar 5: Safe Elimination


If you are constipated, you cannot effectively clear the toxins you are trying to detoxify.


The Essential: PHGG (Partially Hydrolyzed Guar Gum)


  • Why: This is a soluble fibre that is exceptionally well-tolerated by people with SIBO and IBS. It improves motility and supports healthy gut flora without causing the typical "fibre bloat."


  • My Recommendation: Micronutrition PHGG.


A note on diet

While preparing your body, I recommend reducing obvious "High-FODMAP" triggers like onions, garlic, and beans.


However, avoid a fully restrictive SIBO diet during this prep phase.


This is because staying on a restrictive diet for too long can adversely affect your microbiome diversity and actually make constipation worse. Once you start the antimicrobial phase then introduce a low fodmap/SIBO.
